(If your income exceeds 55k for married couples or 32k for a single person)<\/li>\n\n\n\n<li>List of any additional resources the applicant has already applied for. (GoFundMe, help from family and friends, payment plans, etc)<\/li>\n<\/ul>\n\n\n\n<p><strong>Vet Clinic Information:<\/strong><\/p>\n\n\n\n<ul>\n<li>A copy of the written estimated cost for the needed treatment with proof of good prognosis as determined by a veterinary professional at the treating clinic.&nbsp;<em>This does not apply to those seeking assistance from the Pet Supply Fund.<\/em><\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>We require the following documentation.
