We require the following documentation.

Financial Information:

• Proof of Income – This can be copies of two recent paystubs, unemployment benefits, disability benefits, social security income statements, etc for all working adults in the household where the pet lives.
• Proof of any hardship you’re claiming. (If your income exceeds 55k for married couples or 32k for a single person)
• List of any additional resources the applicant has already applied for. (GoFundMe, help from family and friends, payment plans, etc)

Vet Clinic Information:

• A copy of the written estimated cost for the needed treatment with proof of good prognosis as determined by a veterinary professional at the treating clinic. This does not apply to those seeking assistance from the Pet Supply Fund.
